Review Process
From the Initial Care and Education plans devised on
admission, a series of reviews will take place.
Initial Review
This review is arranged at the point of admission and
usually takes place between 7 and 10 days after admission. The local
authority social worker and Moore House Care & Education senior
staff will attend the meeting. The purpose of the review is to discuss
the admission and young person's response, consider the Initial
Personal Plan, and set out the aims of the assessment. The key teacher
and key worker will meet on a weekly basis to review progress and
begin to devise the Individual Care and Education Plans.
6 Week Review
The purpose of this review is to discuss and consider
a more comprehensive care and education plan. This is known as the
Personal Plan and will be presented in draft form at the meeting.
This review is part of the LAC process and will normally be chaired
by the local authority reviewing officer. Following this review
they key teacher and key worker will continue to meet on a weekly
basis to maintain a clear focus and monitor progress. This weekly
meeting will be maintained for the first six months of the placement.
6 Month Review
After the initial 6 week period, the Personal Plan
will be reviewed twice per year at the LAC 6 month review. Again,
the local authority reviewing officer will chair this. The review
will consider progress over the 6 month period and set new targets
for the future. The key teacher and key worker will meet on a monthly
basis thereafter unless the young person moves into a period of
crisis.
