Referral Procedure and Review Process
Referral to Throughcare Services
The Throughcare Service exists primarily to support
young people who have had successful placements within Moore House
Care & Education.
Granby Avenue was opened in February 2003 and Alberta
Avenue was opened in February 2004. Both projects are three bedded
units in the Howden area of Livingston. Individual referrals should
be made to the Throughcare Services Manager in the first instance.
A referral meeting will be arranged where the Social
Worker, local authority Throughcare Worker and any other relevant
agencies will attend. At this meeting a discussion will take place
with regard to the young person's needs. If, at this point, a placement
is offered, this will be confirmed in writing and an authority for
funding will be required.
A further pre-admission meeting will be set up where
the young person and his/her family or carer will be invited to
the project to see around and discuss further the young person's
admission and ongoing development plan. At this point a project
worker will have been identified to act as the young person's individual
support worker and he/she will be present at this meeting as well
as the project leader.
A series of visits, overnights and admissions dates
will be planned thereafter.
A review of the young person's placement will take
place within 7 days of admission to discuss the initial settling
in to the project and the development plan. A review after 6 months
at the project will discuss the progress within the placement and
further development work.
The development plans are flexible and are based on
the Pathways Plan, which will have been completed prior to admission.
