Referral Procedure and Review Process
Education and Care Planning
Education Planning
Each individual young person will have an Individual
Education Plan which will be devised by his/her key teacher on admission.
This will be an informed plan from the young person's background
information from the referral stage. The young person will be placed
in an age appropriate class group and within the first week will
be tested in English and Maths.
After two weeks the young person's ability in a number
of behaviour related areas throughout the school will also have
been tested. After ten days the key teacher will be in a position
to provide detailed information to the initial review.
After six weeks a full assessment will have been completed
through the initial personal plan and from this point a full Individual
Education Plan can be available. This plan will be reviewed every
three months.
Care Planning
On admission to the residential unit each young person
will have a Care Plan devised to suit his/her individual needs.
This will include the following areas:
- Social Presentation
- Family & Social Relationships
- Identity
- Emotional & Behaviour Developments
- Self Care Skills
- Health
The young person's key worker and case manager will
devise the Care Plan as described in the Referral and Admission
procedure. The key worker will also have regular key time with the
young person, both in the school and externally, which will provide
the appropriate time for the two to build up a warm and enabling
relationship which allows the young person to trust his/her key
worker. This will also provide the forum for the key worker and
young person to review the Care Plan.
The school encourages the young people to have a sense
of ownership over their Care Plan and be the central person in its
development and ongoing success.
